CHECK TO SEE IF A CLAIM WAS PAID

 


Click "Enter" at the bottom of this page to check on the status of recent medical or dental claims submitted to the Fund Office. 

Your Social Security number and your Personal Identification Number (PIN) are required. If you have not established a PIN number yet, simply follow the instructions on the next page. You will not be able to access these files if the Social Security number or the PIN number is not valid or does not match our files.

You will have two options on the next page:

1. Claims Paid: Displays the last claims paid for the Social Security number you entered.  You will see the following information:

  • Claimant's first name
    (The name of the family member who incurred the claim)
  • Beginning and ending dates of service
  • Claim "document number"
    (Used by the Fund Office for identification)
  • Total charged amount
  • Deductible taken, if any
  • Check amount
  • Check date (The check is always mailed on the next business 
    day following the check date)
  • Check number
  • Provider name

2. Claims Pending: Displays claims received but not yet paid. 
  • Claimant's first name
    (The name of the family member who incurred the claim)
  • Beginning and ending dates of service
  • Claim "document number"
    (Used by the Fund Office for identification)
  • Total charged amount
  • Received date 

  • Status (If a letter was sent requesting information, you will see "Req:" followed by a short description of the information requested.
  • Provider name
