your California Vacation-Holiday account.
The columns you will see
on the next page from left to right are:

Wkpd – This refers to
the work month during which hours were reported by your employer;

Process Date – This may
be:
-
Date your employer’s
report was processed; or
-
Date a lien against
your account was processed; or
-
Date we issued you a
withdrawal check from your account. If a check was issued to you, the check
number will also appear.

Employer – Name of your
employer (or other action described).
-
Hours – The hours
reported on your behalf by your employer.
-
Contrib – The total
amount contributed on your behalf by your employer to the Vacation-Holiday
Savings Trust.
-
Dues – The amount paid
to Local 12 for Supplemental Dues.
-
Net – The difference
between the total contribution amount and the Supplemental Dues amount.

Balance – The running
total from the “Net” column after additions and subtractions.
At the bottom of the list
you will find column totals and the amount currently available for withdrawal
from your account.
Your Social
Security or Member ID number and your Personal Identification Number (PIN) are required.
If you have not established a PIN number yet, simply follow the instructions on
the next page. You will not be able to access these files if the Social Security
number or the PIN number is not valid or does not match our files.
